The 23rd Student Life Questionnaire

Osaka University will conduct a student life questionnaire in October. What's the purpose of doing this? By conducting such a questionnaire, the university hopes to get a better understanding of students' lives and things which students think should be improved. In other words, the university plans to use the questionnaire  results for improving student welfare, extracurricular activities, and facilities. Please assist the university by participating and filling out the questionnaire carefully.

Survey period : October 1 - October 31, 2013

Application deadline : The original application deadline of Thursday, October 31, 2013 has been postponed to Thursday , November 7 , 2013 . Please fill out this form and put it into the box in the Student Centers. (updated: October 25, 2013)

Presents : Two respondents will be given an iPad mini, four people, a Kindle Paperwhite, and 100, book coupons worth ¥2,000.
