Notification of a change of name, date of birth, gender, or nationality/region
Notification is required for changes to personal information such as your name, date of birth, gender, or nationality/region (e.g., due to marriage or other circumstances).
Notification period: Within 14 days of the change
Procedure: Notify the change to a regional immigration services bureau. This notification, in principle, must be filed in person at the bureau by the individual themselves, but if that person is under the age of 16 or unable to do so due to illness or other reasons, a family member living with said individual must file the notification on their behalf
Required documents:
- Passport
- ID photo (4cm x 3cm)
- Residence card (or foreign resident registration card)
Notification of an affiliated organization
For mid- to long-term residents (those granted a period of stay exceeding three months) with work-related statuses such as "Engineer" (excluding "Artist," "Religious Activities," and "Journalist") or study-related statuses such as "Student," you are required to notify the regional immigration services bureau if any of the following occur.
- Your affiliated organization (employer or educational institution) changes its name or address, or ceases to exist
- You resign due to contract expiration
- You transfer to a new organization due to a new contract
Notification period: Within 14 days of the change
Procedure: Visit your regional immigration services bureau in person or submit a notification by mail to the Tokyo Regional Immigration Services Bureau.

Notes
Enclose a copy of both sides of your residence card along with your notification. Please write “NOTIFICATION ENCLOSED” in red ink on the front of the envelope.
Please send your notification via registered mail or another trackable method, as the bureau does not provide confirmation of receipt for mailed notifications.
