Procedures for Information Disclosure

Flow of Information Disclosure Description
Applicant 1. Identification of document to be disclosed The applicant chooses a document that he/she applies for disclosure by viewing University Document Files or by making an inquiry to the Information Disclosure Service counter.

2. Submittal of Information Disclosure Request Form

(Payment of disclosure request fee)

The applicant fills out Information Disclosure Request Form and submit it with a disclosure request fee (300 yen per document) to the Document Management Office,  General Affairs Division. If he/she requests the copy of the document, sends the fee by registered mail or bank transfer (postal money orders also accepted).

3. Receipt of Disclosure Request Form
4. Correction of Disclosure Request Form If there are flaws in the request form, Osaka University asks the applicant to correct the form. The period of correction will not be included in the 30 days after the date of determination.
5. Identification of the document
6. Inquiry to the third party In case information on the third party (corporates, etc.) is included in the identified document, depending on the content of information, Osaka University asks the third party for opinion on the disclosure.
7. Determination on disclosure of the document by the President of Osaka University

After hearing opinions from the Information Disclosure and Personal Information Protection Committee

8. Receipt of a notice of determination of information disclosure
9. Submittal of Disclosure Method Form
(Payment of a disclosure implementation fee)
The applicant submits Disclosure Method Form within 30 days after the date of determination.
If President of Osaka University deems unappropriate to ask the applicant to bear the fee for economic reasons, the fee may be reduced or the applicant may be exempted from payment of the fee.
10. Disclosure implementation
(Viewing/receiving a copy of document)
The applicant views the document or receives a copy of such. If he/she hopes the copy of such, enclose the appropriate postage as a shipping charge when making application for disclosure.

Applicant 11. Application for further disclosure The applicant can receive copies of documents that he/she viewed by submitting Application Form for Further Disclosure. In this case, disclosure request fee is not necessary; however, a disclosure implementation fee is needed.

 

Application for Review

When there is an objection to a decision on non-disclosure or partial disclosure, the applicant can file an application for review with the President.

Osaka University 2.Information Disclosure and Personal Information Protection Committee

Osaka University asks the Information Disclosure and Personal Information Protection Committee for opinions and then asks the Information Disclosure and Personal Information Protection Review Board of the Ministry of Internal Affairs and Communications for advice.

Osaka University 3.Following a recommendation by the board, OU will make a decision.

Following a recommendation by the Information Disclosure and Personal Information Protection Review Board of the Ministry of Internal Affairs and Communications, Osaka University will make a final decision.
